A dental onlay is a custom-made cover that repairs a tooth with damage too large for a normal filling, but not large enough to need a full crown. It covers one or more of the chewing points, called cusps, on top of the tooth. Onlays are usually made of ceramic or a strong composite material and are fixed in place with dental cement.
| Filling | Onlay | Crown | |
|---|---|---|---|
| Used for | Small to medium cavities | Larger damage, including part of the chewing surface | Major damage or a weak whole tooth |
| Tooth structure removed | Least | Moderate | Most |
| Made | Directly in the mouth, same visit | In a lab or with a scan, needs 2 visits | In a lab or with a scan, needs 2 visits |
| Strength | Good for small areas | Strong, protects remaining tooth | Strongest, covers the full tooth |
An onlay saves more of your natural tooth than a crown, while giving more strength and a better fit than a large filling would in the same spot.
Dr. Urvi Patel will tell you honestly if a filling, onlay or crown is the right fit for your tooth, based on how much of it is damaged.
This usually takes 2 visits, a few days to a week apart, depending on where the onlay is made.
With good care, ceramic and composite onlays commonly last 10 to 15 years or longer.
The cost depends on the material chosen, the size of the onlay, and the tooth being treated. A written estimate is given after examination.
No. A filling is placed directly in the tooth in one visit. An onlay is custom-made outside the mouth to fit the exact shape of the damage, which usually needs two visits.
No. A crown covers the entire tooth. An onlay covers only the damaged part, including the chewing surface, so more of your natural tooth is kept.
The tooth is numbed before any preparation, so you should not feel pain during the procedure.

Sometimes, depending on how much tooth structure remains. In many cases a crown is preferred after a root canal on a back tooth. Your dentist will advise based on your specific tooth.
